The Medical Outreach - Indigenous Chronic Disease program increases access to a range of health services for Indigenous people, including expanded primary care in the treatment and management of chronic disease. There are 22 different types of specialist providers currently delivering services across 16 locations throughout South Australia.
The program focuses on addressing the management of chronic diseases including diabetes, cardiovascular disease, chronic respiratory disease, chronic renal disease and cancer.
